PCCUA STUDENT WITH DISABILITIES CHECK LIST
I am aware of my responsibilities regarding a disability and have taken the following steps to assure success at PCCUA:

 ____Informed the college at the time that I began  the registration process that I
          have a disability.

____Informed the advisor with whom I have been placed that I have a disability.

____Completed a student Disability Form

____Requested accommodations  from my advisor at pre-registration or within
         the first two weeks of classes.

____Provided the college with the documentation necessary for me to receive
         accommodations for my disability.

____Received an Instructor Accommodations Form which I have taken to each
         instructor  informing them of my disability and the accommodations I need
         to do my work.

____Submitted the Instructor Accommodations Forms signed by me, the
         instructors teaching me, and my advisor.

____Received a copy of my accommodation form for my records.

____Meet regularly with my advisor.

____Requested tutoring if it is needed.
